An MQ-9A departed the runway at an undisclosed location and crashed on landing Sept. 3, 2018. The Abbreviated Accident Investigation Board President found, by the preponderance of the evidence, the cause of the mishap to be the design of the engine.

Headquarters Air Education and Training Command released its Accident Investigation Board report May 1, 2019, for the T-6A Texan II aircraft incident that happened near JBSA-Randolph Sept. 18, 2018. At 3:40 p.m. that day, during an instructor qualification flight, the T-6A’s engine failed outside the distance required to glide to a suitable landing surface. The two-person aircrew safely ejected from the aircraft and suffered only minor injuries.
